This form is used to transfer ownership of a total-loss vehicle to USAA.

• Please print one Total Loss Power of Attorney form for each owner listed on the front of the vehicle's title.
• The name(s) must be printed and signed exactly as printed on the front of your title. If you are unsure as to the exact spelling or name(s) on your title, contact your lien holder or your local department of motor vehicles to obtain this information. If you cannot determine the name(s) exactly as on the front of the title, contact your total-loss representative before completing the form. A notary is required only in certain states. Your total-loss representative will let you know if your form must be notarized.
• You can obtain the vehicle identification number from USAA's Auto Policy Summary.
• Once completed, please send the original power(s) of attorney to the following address:

• Faxed copies are not acceptable
• Any erasures, strike-throughs or alterations to the power of attorney may void the document and may not be accepted by the department of motor vehicles. This will require you to submit a new power of attorney.
